Objectives

 

After the completion of this rotation, a resident shall be able to:

 

1.        Perform a history and physical examination in order to diagnose common sleep disorders, including obstructive sleep apnea, narcolepsy, periodic limb movements, delayed sleep phase syndrome, chronic insufficient sleep syndrome and insomnia.

 

  1. Describe the expected sleep architecture in the disorders listed in objective #1.

 

  1. Describe how sleep architecture changes from birth through old age.

 

  1. Develop a differential diagnosis for patients who present with excessive daytime sleepiness

 

  1. Understand the approach the diagnosis and evaluation of patients with chronic insomnia

 

  1. Understand the relationship between sleep and learning, and the relationship between sleep disordered breathing and ADHD

 

  1. Understand the differences between adult and pediatric obstructive sleep apnea

 

  1. Understand the indications for polysomnography, multiple sleep latency testing, and maintenance of wakefulness testing.

 

  1. Prescribe a rational therapeutic plan for patients with the disorders listed in objective #1

 

 

Methods

 

1.                    Attendance at new patient evaluation clinic 9AMnoon every Tuesday and Thursday

 

2.                    Attendance at followup clinic 1 – 3 PM each Monday and Wednesday

 

3.                    Observation of polysomnographic recording once during rotation

 

4.                    Review of polysomnographic recordings – at least two sessions with sleep specialists

 

5.                    Attendance at weekly case review with Drs. Makki and MacDonald, and regional case conferences

 

6.                    Attendance at monthly staff meetings

 

7.                    Attendance at lectures provided by Drs. Makki and MacDonald

 

8.                    Assigned readings pertaining to patients evaluated in center from:

 

Kryger M, Roth T, Dement W, Ed.  Principles and Practice of Sleep Medicine.  WB Sauners, Philadelphia, 3rd Ed., 2000. (available in Sleep Disorders Center and Sinai Grace Hospital library).

 

Residents are also expected to attend Sinai Grace Hospital Department of Medicine morning report ( 9: 30 AM Mon, Tues, Wed, Fri), Grand Rounds (Thursday 9 AM), and noon conference on a daily basis.
